(I suggest to always use the short formats because the long month name has
to be in grammatical 2nd case - genitive. This is something MS didn't even
get to work in Windows.)

Names in Lithuanian postal addresses are in the grammatical 3rd case
(dative). A person with the name Donatas Jonaitis would be mentioned like in the following example:

The Lithuanian abbreviation for PO box is a/d. But instead that, PO Box can
be used, too.

Please keep in mind that some formats (writing of phone numbers and postal
addresses) are not fully standardised and that some people prefer formats of
their own. Nevertheless these are the formats generally recognised and
mostly used.
